New Patients: What You Will Need

When you come to the clinic for the first time, one of our team members will ask you for the following:

Please print the “New Patient Forms” and take a few minutes in the comfort of your home, to carefully read and answer all of the questions.  Then, bring the New Patient Forms filled out with you on your first visit. Also, please take a moment to read our Notice of Privacy Practices.

What to Expect

You will be evaluated by one of our licensed Physical Therapists. This evaluation will include, but not be limited to, checking the motion, strength, muscle flexibility of the affected and surrounding areas

Based on these findings, your Physical Therapist will develop with you a plan of care to “help you feel like you again”.

The plan of care may include modalities such as ultrasound, electric stimulation, heat/ice, whirlpool, traction, iontophoresis, and hands on techniques of massage, mobilizations, stretching, myofascial release, and an exercise program, all tailored to your needs.

You may experience some level of soreness following the evaluation, which is normal, as we determine the cause of your pain or problem to better treat you.

The first visit will last approximately 45 minutes to 1 hour, with subsequent visits averaging around 40-45 minutes (depending on your plan of care).

 

Things Every Patient Should Know
Before Starting Physical Therapy

  1. Direct Access is available in Connecticut.  This means you can go directly to your Physical Therapist without seeing your Doctor first (*some restrictions apply).  Call our office for more details.
  2. Your evaluation should only be performed by a licensed Physical Therapist, not by a Physical Therapist Assistant, Athletic Trainer, etc, as they are not fully trained to perform the evaluation and develop a diagnosis and treatment plan.  It is a Federal Law that they not perform these.
  3. Do your exercises.  Research shows that most patients respond much better when clinical therapy is combined with home exercises.
  4. Embrace the goals of your therapy.  Your Physical Therapist’s intent is to help you regain your independence, educate you about your condition, share self-treatment methods, and emphasize a strong wellness component for injury prevention in the future.
  5. To ensure strong communication, always ask you Physical Therapist any questions you have regarding your care.
  6. Always feel free to contact the front office staff for questions regarding your health insurance or payment issues.
